Enliven Medical Social Work Services
Enliven Home Health provides Medical Social Work services to patients requiring assistance with social or emotional problems that are impacting their health and /or medical treatment. The Social Worker is also available as a resource to Enliven staff and patients to consult and provide guidance on other social and /or financial issues that may be present during the course of treatment.
Medical Social Services program includes (but is not limited to) the following:
- Assessment of Social and Emotional Factors
- Counseling for Long-Range Planning and Decision Making
- Community Resource Planning
- Referral to Community Programs, Services and Agencies
Social Work professionals can provide educational presentations and resources on request and are available to answer questions when needed.
