Opportunity: Medical Assistant – Full Time (Monday – Friday)

We are a Home Health service provider seeking a highly motivated Medical Assistant to join our Extraordinary Team!

Responsibilities:

  • Coordinate with staff, Healthcare providers and Referral sources to assure continuity of care and adherence to the plan of care.
  • Answering phone and triaging calls proficiently and with the highest level of professionalism
  • Assists in Insurance Eligibility verification
  • Assist in Clinical scheduling for all disciplines
  • Assist in sending and receiving physician orders timely and in compliance with our policy’s and procedure’s.
  • Ensure front office and reception area clean and tidy
  • Greets visitors in a friendly and welcoming manner. Creates a positive first impression. Smiles, projects a positive attitude.
  • Assist in the creation of patient’s charts
  • Assist in the Data entry of all new referrals
  • Ensures compliance with all state, federal, referral/intake regulatory requirements
  • Ability to adhere to HIPAA Standards
  • Other duties as assigned by Administrator or Director of Clinical Management Coordination

Qualifications:

  • Completion of Medical Assistant course
  • Fluent in Medical Terminology
  • Strong communication and critical thinking skills
  • Must have the ability to multitask in a fast-paced Home Health office
  • Ability to work in a highly collaborative team environment

Opportunity: Physical Therapist (PT)

Qualifications:

  • Physical Therapist License for Connecticut (Required)
  • Strong clinical skills (Prior Home Health Experience Preferred)

Summary:

Work with a multi-disciplinary team of clinicians providing evaluation, treatment planning and ongoing care for homebound patients often in an assisted living settings. Physical therapist will follow their client from evaluation to discharge.

Responsibilities:

  • Provide comprehensive rehabilitation services to members of the community in Connecticut.
  • Performs a comprehensive assessment including physiological, psychological and environmental factors. Utilizes appropriate techniques and materials.
  • Encourages and promotes patient/family independence.
  • Develops, evaluates, and revises patient care goals in conjunction with the patient and family.
  • Coordinates goal directed, cost effective care for primary patients and demonstrates appropriate use of community resources to deliver comprehensive individualized care.
  • Communicates with MD, family and interdisciplinary care providers to clarify care and increase awareness of significant changes in condition in a timely manner.
  • Maintains effective working relationships with patients/families, referral sources, physicians, community resources and insurance representatives.
  • Provide patient and family with appropriate information and materials necessary to support the learning process, i.e., home exercise program sheets and instruction sheets.

This position will allow you to utilize and develop your skills in case management, falls prevention, specialty equipment ordering, wound care and more.

You will have a flexible schedule and utilize an electronic EMR which will help limit travel time and visits to the office.
